Valproic Acid + Meropenem
Mechanism: Carbapenems (Meropenem) can rapidly and significantly decrease serum concentrations of Valproic Acid, potentially leading to loss of seizure control. Mechanism involves inhibition of intestinal absorption, increased hepatic glucuronidation, and enhanced renal excretion.
Clinical Action: Concurrent use is generally avoided. If unavoidable, monitor VPA levels closely and consider alternative AED or antibiotic.
